SMA’s pharmacy is incomparable. Embedding a clinical pharmacist within our practice has created collaborative relationships among our patients, clinicians, and retail pharmacists. Our clinical pharmacist increases efficiencies by supporting the reconciliation of medications, implementation of protocol-driven changes in pharmacotherapies, and drug substitutions to increase safety and/or lower costs.

Clinical pharmacists provide direct medication management improved coordination of care, increased patient education and advocacy, and triage and referral services.

SMA’s clinical pharmacy includes services include prior authorizations, refills, drug monitoring programs for drugs with a narrow therapeutic index, and patient education, including reconciliation and counseling.  Our team also provides support to patients who have difficulty managing their diabetes, cholesterol levels, and blood pressure. Working with both our pharmacist and physician, patients can create a regimen designed to optimally manage acute or chronic disease.
